Explanation of Agreement Types

 

Full Service:
Children and youth who live in districts under this plan can make appointments on their own (parent referred). No referral from the district is needed. Follow-up information can be sent to the school if requested and the student or parent signs the appropriate releases. Students can be seen for substance use or mental health issues regardless of their insurance coverage. No fee is charged to the student under this agreement. Families will receive a discounted rate on select Youth Programs.   
 
School Referral Only:
Children and youth who live in districts under this plan must be referred from a person/persons designated by the district.  Follow-up information will be released to the designee when requested and appropriate releases are signed by student/parent.  Children and youth can be scheduled for both substance use and/or mental health issues.  Students who are referred through the designee are not charged.
 
Children and youth from this district who are not referred from the appointed designee are considered a community referral and will be charged on the sliding fee scale.  No follow-up is provided.  Children and youth who have health insurance will be referred to a Student Assistance Counselor for screening.
 
 
Community Referral for Substance Use Only:
Children and youth who live in districts that do not have a formal agreement are self-referred (non-school). These referrals will pay a fee based on a sliding scale. If the child has health insurance, the child may be instructed to receive services directly from his or her health care plan. No follow-up information is available to school personnel.
 
Children who have a valid Medicaid card are entitled under their Medicaid coverage to receive a substance use assessment at CARE. No school follow-up is provided under this coverage.
 
Children and youth can be seen for substance use issues ONLY. Mental health concerns will be referred to Community Mental Health
 

CARE’s Student Assistance is funded by local school districts, the DaimlerChrysler Corporation Fund, the Detroit Newspaper Partnership, L.P., the Detroit Free Press and fundraising efforts.
